I have rewritten a considerable number of forms and templates for the department (and grad students) into LaTeX code. Below is a master list of the pdfs as well as the raw .tex files for these. Keep in mind that some of these are for official use only and you should get permission from the Chair (currently Kevin Knudson) to use any of the following files that are marked with an asterisk. Many of the forms I make include some level of automation to make it possible to compile a pdf that is fully filled out with a TeX compiler, to avoid having to print the pdf, fill it out by hand, scan it, and then email it. When that level of automation is included, I have made a note. For some of these they require an official UF signature file (the official UF logo) which can be found here.
